The Social Worker will provide therapeutic interventions and psychosocial assessments to support residents' emotional and social well-being within a skilled nursing environment. They are responsible for coordinating interdisciplinary care plans, facilitating discharge planning, and acting as a liaison between residents, families, and external agencies.
Candidates must hold a Master's degree in Social Work or a Human Services field and possess a valid CSW or LSW license in New Jersey. At least 1-2 years of professional social work experience, preferably in long-term care or skilled nursing, is required.
