This position offers day shifts only. Work with Beloit Health System in Beloit, Wisconsin.
The Care Coordinator will conduct psychosocial assessments and develop individualized discharge plans to facilitate safe transitions of care for patients. They will also provide crisis intervention and coordinate community resources while collaborating with an interdisciplinary healthcare team.
Candidates must hold at least a Bachelor's Degree in Social Work and possess certification to practice in the State of Wisconsin. Strong skills in care coordination, documentation, and crisis intervention are required for this role.
