This position offers weekends off. Work with Sanctuary Hospice in Rosemont, Illinois.
The social worker will provide compassionate counseling and psychosocial support to hospice patients and their families. They will also collaborate with the interdisciplinary team to develop and implement individualized care plans.
Candidates must hold a Master of Social Work degree and a valid state license as a social worker. A minimum of one year of experience in a healthcare or hospice setting is required, along with a valid driver's license and reliable transportation.
