Key Responsibilities

Conduct in-home health evaluations for health plan members to bridge gaps in care and provide personalized health assessments. Perform medication reviews, physical evaluations, and diagnostic tests while maintaining accurate clinical documentation.

Requirements

Must be a licensed physician (MD or DO) with an active, unrestricted license in the coverage area. Ability to travel up to 100% of the time and obtain multi-state licenses is required.
