Key Responsibilities
The Nurse Practitioner will provide advanced palliative care to patients in their homes, collaborating with the Palliative Medical Director to develop and implement individualized care plans. Responsibilities include performing comprehensive patient assessments, managing treatment plans, and facilitating referrals for additional care services.
Requirements
Candidates must hold a Master of Science in Nursing and have at least two years of Nurse Practitioner experience, preferably in Family or Adult Medicine. A current RN license, Nurse Practitioner certification, and a valid driver's license are required, with ACHPN certification preferred.
