Key Responsibilities

Coordinate patient care for an assigned caseload, including developing and implementing patient-specific plans of care and supervising PTA staff. Conduct physical assessments, document clinical notes, and communicate with physicians and interdisciplinary teams to ensure quality outcomes.

Requirements

Requires a current Massachusetts Physical Therapist license and active American Heart Association BLS certification. Candidates should have 1-2 years of experience, preferably in home health, and proficiency in OASIS documentation.
