Key Responsibilities
The RN Care Navigator coordinates and delivers patient care while conducting Medicare annual wellness visits and patient outreach. They are responsible for educating patients, scheduling appointments, and escalating clinical concerns to providers within an ambulatory setting.
Requirements
Candidates must be a graduate of a nursing school with a current, unrestricted Registered Nurse license in the State of Ohio. A minimum of two years of ambulatory experience and one year of case or care management experience is preferred.
