The Case Manager provides direct nursing care, conducts comprehensive home health assessments, and coordinates patient services. They are also responsible for maintaining accurate patient records and supervising home health aides and LVNs.
Candidates must be a graduate of an accredited nursing program and hold a valid Texas Registered Nurse license. At least two years of RN experience, including one year of adult IV expertise, is required.
