The Physical Therapist will perform home health visits to provide rehabilitative care, including mobility assessments, gait training, and individualized care plan development. They are responsible for educating clients and families while accurately documenting all interventions and evaluations using electronic systems.
Candidates must hold a current Virginia Physical Therapist license and have at least one year of recent work experience. A degree from an accredited physical therapy program is required, along with strong organizational and computer skills.
