Key Responsibilities

• Build the Function.

• Stand up pharmacist-led medication management workflows across core programs.

• Personally manage a panel of patients to establish the clinical and operational gold standard.

• Design protocols for medication initiation, titration, monitoring, and adherence.

Requirements

• PharmD required; active pharmacist license in DC, MD, VA, GA, TX.

• 5–8+ years of clinical pharmacy experience, ideally in cardiology, ambulatory care, or population health.

• Experience with heart failure management and chronic disease management.

• Prior experience building or scaling programs.

Work Schedule

• Full-Time position.

• Remote position - work from anywhere.
