Key Responsibilities
The Master Social Worker provides psychosocial services to dialysis patients, collaborating with the interdisciplinary team to improve patient quality of life and treatment outcomes. Responsibilities include conducting patient assessments, facilitating care planning, providing counseling, and assisting with insurance and community resource referrals.
Requirements
Candidates must hold a Master's degree in Social Work and possess the required state license. A minimum of 2 to 5 years of related experience is required for this position.
