Job Description
As part of our dynamic interdisciplinary palliative medicine team, you'll make a real difference in patients' lives by providing comprehensive palliative medicine consultations to those navigating serious illness in the inpatient setting. Here's what you'll be doing:
Conduct comprehensive assessments that honor the whole person—diving into physical, psychological, and spiritual concerns for patients with serious illness and their families.
Facilitate meaningful advance care planning conversations and guide patients through advance directive completion, helping them clarify what matters most.
Master pain and symptom management with skill and compassion, ensuring patients with serious illness receive thoughtful, personalized care.
Collaborate with the interdisciplinary team to lead impactful goals of care discussions, working alongside the primary team to ensure patient voices are heard.
Champion emotional wellness by recognizing and seizing opportunities for health counseling with patients and families, partnering with the team to address the emotional needs of patients and their caregivers.
Share your expertise by educating other clinicians, healthcare administrators, and patients/families about hospice and palliative care concepts—spreading knowledge that transforms care.
Maintain thorough clinical documentation that captures the complete picture: patient condition, care provided, symptom management updates, and advance care planning discussions.
For NPs: Obtain or maintain current certification as a Certified Hospice and Palliative Nurse (CHPN) or Advanced Certified Hospice and Palliative Nurse (ACHPN) from the Hospice and Palliative Nurses Association (HPNA)—or commit to earning it within 2 years of joining us.
Stay sharp and inspired by participating in continuing medical education related to palliative medicine.
Bring your APP experience to the table—whether you're a seasoned palliative medicine APP or an experienced clinician ready to transition into this specialty, we'd love to have you on board.
