Key Responsibilities
The Assessment Nurse Case Manager conducts face-to-face home visits to perform health risk assessments, PCA assessments, and MDS submissions for new and existing enrollees. They collaborate with the care team to provide member education and ensure compliance with regulatory and contract requirements.
Requirements
Candidates must be a Registered Nurse with an active, unrestricted license in Massachusetts and at least one year of clinical experience with complex medical, behavioral, and social co-morbidities.
