Assessment Nurse Case Manager

Key Responsibilities

The Assessment Nurse Case Manager conducts face-to-face home visits to perform health risk assessments, PCA evaluations, and MDS assessments for new and existing enrollees. They collaborate with the care team to provide education, advocate for members, and ensure compliance with regulatory requirements.

Requirements

Candidates must be a Registered Nurse with an active, unrestricted license in Massachusetts and at least one year of clinical experience with complex medical and social co-morbidities. A valid driver's license, reliable transportation, and the ability to conduct in-person assessments are mandatory.
