Per Diem Physical Therapist Assistant (PTA) Home Health- Lake County

Key Responsibilities

Provide skilled physical therapy treatments and follow established plans of care to help patients improve mobility and independence in their homes. Document patient responses and collaborate with an interdisciplinary team of nurses and physicians.

Requirements

Requires a current Illinois Physical Therapist Assistant license and CPR certification. A minimum of one year of home health experience and familiarity with Medicare regulations are preferred.
