About the Role

Nest Home Care LLC is looking for a passionate and dedicated Occupational Therapist to join our growing team in Owings Mills, MD. This is an exciting opportunity to make a meaningful difference in patients' lives by helping them regain independence and improve their quality of life — right in the comfort of their own homes.

Responsibilities

Evaluate patients' functional abilities, daily living skills, and home environments to develop individualized treatment plans

Provide skilled occupational therapy services to home care patients across a variety of diagnoses and conditions

Implement therapeutic interventions focused on improving ADLs, fine motor skills, cognitive function, and safety

• Recommend and train patients and caregivers on adaptive equipment and home modifications
• Collaborate with interdisciplinary care teams including physicians, nurses, and physical therapists
• Maintain accurate and timely clinical documentation in compliance with regulatory standards
• Monitor patient progress and adjust treatment plans as needed to meet goals

Requirements

• Master's or Doctoral degree in Occupational Therapy from an accredited program
• Current Maryland state licensure as an Occupational Therapist (or ability to obtain)
• NBCOT certification required
• Home health or home care experience preferred
• Strong knowledge of ADL training, adaptive equipment, and home modification strategies
• Excellent communication, time management, and organizational skills
• Reliable transportation and valid driver's license for home visits in the Owings Mills, MD area
