Compensation and Benefits
Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses) Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays. Key Responsibilities Deliver person-centered care using evidence-based practices Collaborate with interdisciplinary facility teams Conduct assessments, develop treatment plans, and manage follow-up care Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs) Educate patients, families, and staff Complete timely documentation (within 24 hours) Compliance with facility, state and federal regulations/protocols.
Qualifications
Qualifications: Active license as a Nurse Practitioner (NP) Minimum 2 years of post-certification experience Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred Proficient with EHRs and clinical technology Strong communicator with a collaborative mindset
Key Responsibilities
Deliver person-centered palliative care and chronic care management within skilled nursing facilities. Collaborate with interdisciplinary teams to conduct assessments, develop treatment plans, and maintain timely clinical documentation.
Requirements
Requires an active Nurse Practitioner license and a minimum of 2 years of post-certification experience. Experience in SNFs, hospitalist roles, emergency departments, or ICU is preferred.
