What You’LL Do

Comprehensive Primary Care

* Provide outpatient primary care services for patients across the lifespan (Family Medicine or Med-Peds scope).

* Diagnose and treat acute and chronic medical conditions.

* Conduct annual wellness visits, preventative screenings, and routine physicals.

* Develop individualized treatment plans focused on long-term health outcomes.

Patient Education & Preventative Care

* Counsel patients on preventative health, lifestyle changes, and disease management.

* Promote continuity of care through proactive follow-up and patient engagement.

* Build strong relationships with patients and families within the local community.

Care Coordination & Collaboration

* Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health, and specialty teams.

* Coordinate referrals, diagnostics, and follow-up care as needed.

* Participate in a team-based, patient-centered care model.

Clinical Documentation & Compliance

* Maintain accurate and timely documentation in the EHR system.

* Ensure compliance with all clinical, regulatory, and quality standards.

* Support quality initiatives and value-based care programs.

What You Bring

Required Qualifications

* MD or DO from an accredited medical school.

* Board Certified or Board Eligible in:

* Family Medicine or

* Internal Medicine-Pediatrics (Med-Peds)

* Active New Jersey medical license (or ability to obtain prior to start).

* DEA license and CDS registration (or ability to obtain).

* Strong commitment to patient-centered, culturally competent care.

Preferred Qualifications

* Experience working in diverse or underserved communities.

* Experience in outpatient primary care settings.

* Value-based care or population health experience a plus.
