Responsibilities

Perform Physical Therapy evaluation and development of the plan of care appropriate for each patient based on the patient’s physical, mental, emotional, and developmental impairments and functional limitations. A variety of diagnoses and settings served include but are not limited to the 3 outpatient locations.

Provide Physical Therapy interventions in accordance with established standards of care, policies, procedures, and state/federal law.

Supervise Physical Therapist Assistant and Rehab Aides.

Supervise and complete appropriate documentation for Physical Therapy and Physical Therapy Assistant students as assigned by CCCE.

Complete documentation in electronic medical record in a timely manner in accordance within department and professional expectations demonstrating the need for skilled interventions.

Participate in patient care conferences/ staffing as appropriate for multidisciplinary patients.

Qualifications

Education

• Bachelor’s degree from an accredited Physical Therapy program prior to 2005
• Master’s degree from an accredited Physical Therapy program between 2005-2019
• Clinical Doctorate from an accredited Physical Therapy program 2020-Present

License(s)/Certification(s)

• Current IA and IL Physical Therapy licensure (or eligible) upon hire
• Current CPR certification.

• Current CPR certification.
