Case Manager, RN

Key Responsibilities

The RN Case Manager coordinates patient care, performs utilization reviews, and manages discharge planning to ensure efficient use of hospital services. They work within a multidisciplinary team to facilitate patient throughput and achieve optimal clinical outcomes.

Requirements

Candidates must hold a current California RN license and a BLS provider card, with Case Management certification preferred. The role requires at least two years of acute care case management experience or five years of recent acute care nursing experience.
