The Behavioral Health Case Manager provides therapeutic behavioral services, service planning, and treatment coordination for clients. They are responsible for facilitating linkages to resources, crisis prevention, and maintaining positive relationships with outside contacts.
Candidates must be at least 18 years old with a high school diploma and three years of relevant experience, or a Bachelor's or Master's degree in a related field. A valid driver's license and reliable transportation are required for travel within the assigned service area.
