Physical Therapist (PT) - Home Health Services - Full Time (40 hours/week)

Key Responsibilities

Develop and implement physical plans of care based on physician direction and patient assessments. Coordinate services with the Home Health team and provide palliation for hospice patients to improve mobility and daily living activities.

Licensure & Certifications

Must be a graduate of a certified Physical Therapy program with a current Washington State license and CPR certification. A valid driver's license, reliable transportation, and automobile insurance are required.
