Key Responsibilities

• Develop and implement individualized treatment plans based on patient needs.

• Collaborate with physicians, nurses, PT/OT, respiratory therapy, and case management/social worker for coordinated care.

• Educate patients, families, and medical staff on swallowing precautions, communication strategies, and safe diet recommendations. Patient & Caregiver Education:

• Educate patients and families on home safety based on goals of rehab stay.

Work Schedule

• Full-Time position.

• On-site position.
