Key Responsibilities

The Nurse Practitioner provides value-based primary care to Medicare patients via telehealth, including sick visits, chronic disease management, and annual wellness assessments. They collaborate with a multidisciplinary team to ensure effective care coordination and accurate clinical documentation.

Requirements

Candidates must hold a Master of Science in Nursing or higher and possess a national certification as a Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner. An active, non-probationary state license and the ability to obtain additional state licenses are required.
