Key Responsibilities
The clinician will perform comprehensive diagnostic evaluations and manage longitudinal care plans for patients with dementia and their caregivers. They will work within an interdisciplinary team to provide dyad-centered support and ensure high-quality clinical outcomes.
Requirements
Candidates must be board-certified as a PMHNP or AGPCNP and hold active, unrestricted licenses in OR, WA, ID, and OH. Proven experience in geriatrics, memory care, or palliative care is required, along with the ability to thrive in a remote, high-growth startup environment.
