Sage Group HealthCare Staffing is seeking PT Home Health for travel/contract position in Paducah, KY. Accepting both local and traveler candidates.
Position Details

- Pay Rate: Range $2400 to $2520 Weekly Gross Pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
- Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
- Unit: Home Health
- Shifts available: Days
- Shift timings: 8 Hours; Days, 8.00a - 5.00p
Schedule: 40 hours/week guaranteed, five shifts per week, Mon-Fri
Position Requirements
- Current Physical Therapist experience: 1- 2 years - Required
- Recent/Current Home Health PT experience of at least 1 year within the last 3 years - Required
- Homecare Homebase (HCHB) experience - Preferred
Education, License, and Certification Requirements
- Bachelor of Science in Physical Therapy or Similar - Required
- Current State License as Physical Therapist or Compact if applicable - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description
- Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.
- Observe, record, and report patient reactions, changes, and progress to IDT team
- Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
- Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
- Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
- Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
- Communicate with the PTAs and nurse aides on supervisory visit results.
- Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Territories to cover: Paducah, KY and surrounding areas

Requirements
Requires a Bachelor of Science in Physical Therapy, a current state license, and BLS certification. Candidates must have 1-2 years of PT experience, including at least one year of recent home health experience.
