Key Responsibilities
Provide full-time outpatient primary care to senior patients within a value-based care model. Manage complex chronic conditions such as heart disease and diabetes while collaborating with a dedicated care team to improve long-term patient outcomes.
Requirements
Requires an MD or DO degree with Board Certification or Eligibility in Family or Internal Medicine and an active medical license and DEA registration. Candidates must have at least 2 years of outpatient experience and proficiency in basic office procedures.
