Responsibilities Include
- Assessing patients' social, emotional, and financial needs
- Assisting with community resources and discharge planning
- Providing counseling and emotional support to patients and families
- Collaborating with physicians, nurses, and therapy staff to ensure quality patient care
- Advocating for patient well-being and independence
Qualifications
- Licensed Social Worker (LSW) required; MSW preferred
- Home health experience preferred, but not required
- Excellent communication and organizational skills
- Compassionate, patient-focused mindset
- Reliable transportation and valid driver's license
