Job Description

  • Physical Therapist (PT) – Part-Time
  • Kern River Transitional Care
  • Pay Range: $65.00–$70.00 per hour, based on experience

Kern River Transitional Care is seeking a compassionate and motivated Part-Time Physical Therapist to join our outstanding rehabilitation team. Essential Duties and Responsibilities

  • Evaluate residents’ mobility, strength, balance, coordination, range of motion, and functional abilities.
  • Develop individualized treatment plans based on each resident’s needs, goals, and medical condition.
  • Provide skilled physical therapy services to improve mobility, independence, safety, and overall function.
  • Help residents regain the ability to walk, transfer, maintain balance, and complete daily activities safely.
  • Establish measurable goals and modify treatment plans based on residents’ progress and changing needs.

Provide therapeutic exercise, gait training, balance activities, transfer training, and other appropriate interventions.

Recommend and train residents in the safe use of walkers, wheelchairs, canes, and other adaptive equipment.

Educate residents, families, caregivers, and facility staff regarding treatment plans, safety, positioning, and mobility techniques.

Complete evaluations, progress notes, discharge summaries, and other required documentation accurately and on time.

Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.

Participate in resident care conferences, discharge planning, and quality-improvement activities.

Supervise Physical Therapist Assistants and rehabilitation support staff as permitted by law and facility policy.

  • Maintain residents’ privacy and the confidentiality of all health information.
  • Follow all professional standards, facility policies, safety procedures, and regulatory requirements.
  • Perform additional duties as assigned.

Qualifications

  • Graduate of an accredited physical therapy program.
  • Current and unrestricted California Physical Therapist license required.
  • Current CPR/BLS certification required.
  • Skilled nursing, rehabilitation, long-term-care, or geriatric experience preferred.
  • New graduates are encouraged to apply.
  • Strong clinical assessment, communication, documentation, and time-management skills.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Knowledge of PointClickCare or another electronic medical record system preferred.
  • Compassionate and resident-centered approach to care.
