Las Colinas Post Acute is seeking a skilled and compassionate Registered Physical Therapist (PT) to join our rehabilitation team. This is an excellent opportunity to provide impactful, patient-centered care while working alongside a collaborative and supportive clinical team.

Responsibilities

• As a Physical Therapist, you will:
• Evaluate residents and develop individualized treatment plans
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
• Collaborate with an interdisciplinary team to implement and adjust care plans
• Supervise and support therapy assistants as appropriate
• Accurately document evaluations, progress, and outcomes in compliance with regulatory standards
• Deliver high-quality care with compassion, professionalism, and clinical excellence

Qualifications

• Active Physical Therapist (PT) license in the state of California
• Graduate of an accredited Physical Therapy program
• Strong commitment to patient-centered care
• Excellent communication and teamwork skills
• Experience in a skilled nursing or post-acute setting preferred
