Home Health Physical Therapist (PT) - Contingent

Key Responsibilities

Provides skilled physical therapy treatments in accordance with the physician's plan of care to restore patient function and manage pain. Conducts patient assessments, including OASIS documentation, and instructs patients and caregivers on disease prevention and equipment usage.

Requirements

Requires an active Michigan Physical Therapy license, a valid driver's license, and reliable transportation for home visits. Previous home health care experience is preferred.
