Join Neighborhood Health Association as a Certified Nurse Practitioner in United States. Monday – Friday schedule. No late nights, no weekends, and 11 paid holidays.

Key Responsibilities

  • visit team huddles to review patient schedules, previous notes, lab updates, and needed preventive services. Attend monthly team meetings focused on high
  • risk patients, staffing, role clarification, team building, and quality improvement. Follow the PCMH Patient Flow Chart to streamline care, reduce delays, and improve outcomes and cost

Requirements

  • Active Ohio License: Must hold a current and valid license in the State of Ohio.
  • Certificate of Authority: Required in the applicable field of practice.
  • CPR Certification: Current CPR certification is mandatory.
  • Language Skills: Bilingual proficiency in English and Spanish is preferred.

Work Schedule

  • Full-time position.
