Key Responsibilities

The Master Social Worker provides psychosocial assessments, counseling, and care coordination for hospice patients and their families in a home setting. They also collaborate with the interdisciplinary team to develop and implement patient care plans.

Requirements

Candidates must hold a Master's degree in Social Work and possess a valid social work license. At least one year of hospice experience, a valid driver's license, and AHA CPR certification are required.
