A Brief Overview
Children with heart disease, especially single ventricle heart disease, are prone to blood clots that may have devastating effects. The Cardiac Anticoagulation and Thrombosis Program is a unique, multidisciplinary initiative of CHOP’s Cardiac Center (Cardiology, Cardiovascular Surgery, Cardiac Anesthesia and Cardiac Nursing) and the Hemostasis and Thrombosis Center (Hematology) supported by clinical pharmacy.
The program’s goals are to reduce the incidence and complications of thromboses in cardiac inpatients and to optimize anticoagulation monitoring in cardiac center outpatients. The program includes a strong emphasis on patient safety and monitoring.
In addition to its patient and family–focused care, the program also provides physician and provider education, clinical research and program development. The Children's Hospital of Philadelphia offers anticoagulation medication guidelines for management of pediatric blood clots as well as clinical practice guidelines for the management of common blood clots in pediatric cardiac patients.
