Key Responsibilities

The Physical Therapist will conduct comprehensive patient assessments and create personalized treatment plans to be delivered in the patients' homes. They will also monitor patient progress, adjust treatment plans as necessary, and coordinate care with a multidisciplinary team.

Requirements

Candidates must hold a degree in Physical Therapy from an accredited program and possess a current, unrestricted state license. Proficiency in clinical documentation, compliance, and experience with OASIS are preferred.
