Job Description

QuadMed is looking for a Physician Assistant or Nurse Practitioner to join our
team in our Hampton City School District clinic in Hampton, VA. Join a team that

Schedule: 40hrs/wk

Job Requirements

* Completed master's degree level Nurse Practitioner or Physician Assistant
program from an accredited college/university 
* Minimum three (3) years primary care practice experience  
* Ability to work independently 
* Working with an Electronic Medical Record (EMR)   
* Active and unrestricted applicable licensure 
* Current CPR (BLS) certification or must be obtained within 60 days of hire  
* NPI & DEA  
* Current National Board Certification 
* May be required to obtain and maintain a valid DOT Medical Examiner
Certification, depending on job responsibilities or location

Requirements

Requires a master's degree level NP or PA program from an accredited institution and at least three years of primary care experience. Must hold active licensure, National Board Certification, NPI, DEA, and current CPR (BLS) certification.
