Provide skilled occupational therapy services in patients' homes, including evaluations and the development of individualized treatment plans. Collaborate with care teams and physicians while monitoring patient progress and documenting care electronically.
Requires a current Massachusetts Occupational Therapist license and a minimum of one year of clinical experience. Afternoon availability is required, with preference for those available Monday through Friday or on weekends.
