Key Responsibilities

  • provide short-term, targeted, evidence-based care.
  • Provide short-term, brief interventions using evidence-based approaches (BA, PST, MI, etc.
  • Provide education + self-management support * Track clinical response (in-person, video, or phone) and adjus.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology, or related field
  • Oregon independent license: LCSW, LPC, LMFT, or Licensed Psychologist
  • 3+ years of experience working with adult populations w/ chronic medical + behavioral needs
  • Experience with EMRs + telehealth workflows
