Join Humana as a Field Care Manager, Behavioral Health – Remote, No Weekends in Chicago, Illinois, United States. Become a part of our caring community Humana is looking for a Field Care Manager, Behavioral Health to join the IL Medicaid team. In this position, you will report to the Manager, Care Management and connect with members both face-to-face and telephonically.

Key Responsibilities
The Field Care Manager serves as the primary point of contact for members, providing integrated behavioral health care management and coordinating services to improve health outcomes. They lead interdisciplinary care teams, facilitate care planning, and conduct face-to-face assessments within the community to address health-related social needs.
Requirements
Applicants must hold an active Illinois license as an LCSW, LMFT, or LCPC and possess at least 2 years of post-degree clinical experience in a behavioral health setting. Candidates are required to have a valid driver's license, reliable transportation, and the ability to travel up to 25% within Illinois.
