The Social Worker completes psychosocial assessments for post-hospital care, assists patients with coping related to illness and hospitalization, and collaborates with the Clinical Care Management team to determine appropriate care settings. They also serve as a resource to staff, educate patients on community options, make necessary referrals, and report suspected cases of abuse or neglect.
A Master's in Social Work (MSW) is required, with an LCSW and applicable state certification being preferred. Candidates should ideally possess at least one year of professional experience in a hospital or health-related setting and demonstrate expertise with data management tools.
