Key Responsibilities

The Registered Nurse Case Manager provides clinical care and coordinates multi-disciplinary services to ensure quality, cost-effective patient outcomes. They are responsible for performing assessments, managing care plans, and supervising nurse aides and LPNs while maintaining accurate documentation.

Requirements

Candidates must hold a current Registered Nurse license and have at least one year of experience in an acute care setting. A valid driver's license and auto insurance are required for travel to patient homes.
