Key Responsibilities
The Case Manager will provide comprehensive pediatric and adult home health care, including assessments, wound care, and infusion therapy. They will also play a key role in developing the Pediatric Palliative Care and Hospice program while coordinating care and educating families.
Requirements
Candidates must be a graduate of an accredited nursing school and hold a current Minnesota Registered Nurse license. A minimum of one year of RN experience, a valid driver's license, and an AHA BLS Healthcare Provider card are required.
