Key Responsibilities
The RN Case Manager will deliver hands-on hospice nursing care and coordinate services for patients within the community. They will also collaborate with the Clinical Director to assess patient eligibility and provide ongoing support to families throughout the hospice journey.
Requirements
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in computer programs and EMR systems, along with a valid driver's license and reliable transportation, is required.
