Key Responsibilities

• Must have valid Driver’s License and Personal Auto Insurance Education, License and Certification Requirements: Bachelor of Science degree in Physical Therapy or Similar – Required Current State License as Physical Therapist or Compact if applicable - Required BLS Certification from AHA or American Red Cross - Required Job Description: Provide Physical Therapy services to patients/clients at their homes.

• Provide Physical Therapy services to patients/clients at their homes.

• Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventi.

Requirements

• Required Recent/Current Home Health PT experience: 12 months within the last 3 years

• Required Home Care Home Base (HCHB) charting experience

• Required OASIS experience

• Required BLS Certification from AHA or American Red Cross

Benefits & Compensation

• Salary: $114K - $116K
