Key Responsibilities
Acts as a liaison between members, providers, and insurance companies to coordinate care for catastrophic and chronically ill patients. Develops and monitors individualized plans of care to optimize medical outcomes and facilitate a timely return to work or baseline functioning.
Requirements
Requires an Associate's Degree in Nursing, an active Compact RN license, and at least 2 years of experience in care management and Medicare. Candidates must reside in an Eastern Standard Time (EST) zone state.
