Key Responsibilities

Conduct in-home health evaluations for health plan members to bridge gaps in care and provide personalized health assessments. Perform medication reviews, physical evaluations, and diagnostic tests while maintaining detailed clinical documentation using an iPad.

Requirements

Must hold an active, unrestricted physician license in the coverage area and be capable of obtaining multi-state licenses. Candidates must be comfortable traveling up to 100% of the time and performing duties in diverse home environments.
