Key Responsibilities

The Nurse Practitioner will provide advanced palliative care to patients in their homes, collaborating with the Palliative Medical Director to manage complex medical needs. Responsibilities include performing patient assessments, developing individualized care plans, and prescribing medications while maintaining accurate clinical documentation.

Requirements

Candidates must hold a Master of Science in Nursing and have at least two years of Nurse Practitioner experience. A current RN license, Nurse Practitioner certification, and a valid driver's license are required, with ACHPN certification preferred.
