Key Responsibilities

The RN Case Manager assesses patient needs, coordinates care with the interdisciplinary team, and provides clinical and palliative support in home environments. Requirements

Candidates must hold a current RN license and have at least one year of hospice or hospital nursing experience. A valid driver's license, reliable transportation, and active BLS certification are required to perform home visits and patient assessments.
