Provide physical therapy services to home health patients to restore function and relieve pain. Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans.
Requires a degree in Physical Therapy, a valid state license, and BLS certification. Candidates must have 1-2 years of PT experience, including one year of recent home health experience and proficiency in HCHB charting.
