Key Responsibilities

The Ambulatory Pharmacist optimizes medication therapy within primary care clinics by managing chronic diseases and conducting comprehensive medication reviews. They collaborate with healthcare providers to adjust therapies, interpret lab results, and facilitate seamless transitions of care to improve patient outcomes.

Requirements

Candidates must hold a Doctorate of Pharmacy (PharmD) and a valid pharmacy license in good standing. A minimum of two years of clinical experience in primary care or a related setting is required, with residency training or ambulatory care certification highly preferred.
